Insurance Information

Most orthotic and prosthetic services provided by Lifestyles Orthotics & Prosthetics are covered by health insurance, including Medicare Part B. The amount of coverage depends on your specific insurance plan and the type of device you receive.

 

In many cases, your insurance may require you to pay a portion of the cost out-of-pocket at the time your orthosis or prosthesis is delivered. The amount you are responsible for will vary based on your coverage, any plan maximums, and the device prescribed.

 

Lifestyles Orthotics & Prosthetics works with a variety of insurance plans. If we are out-of-network with your provider, we can still provide services, but you may be responsible for a higher portion of the cost based on your plan’s out-of-network benefits.

 

Once you share your insurance information with us, our knowledgeable staff will verify your benefits with your insurance carrier to determine your coverage. When necessary, we will also obtain prior authorization for your device. Please note that most services require a prescription.
